TITLE OF INFORMATION COLLECTION: WISQARS Home Page Survey

PURPOSE:

WISQARS is an interactive, online database that provides fatal and nonfatal injury data from a variety of sources. People can use WISQARS data to learn more about the public health and economic burden of injury in the United States. Users can search, sort, and view the injury data and create reports, charts, maps, and slides. This information is often used in program planning, implementation, and evaluation at the state level.

The purpose of this project is to obtain information on satisfaction from WISQARS users. The information collected will be used to improve the functionality of the website and develop supplemental resources to help users in interpreting the data.

DESCRIPTION OF RESPONDENTS:

Respondents include individuals that use the Web-based Injury Statistics Query and Reporting System (WISQARS). Based on web metrics and requests for technical assistance, we know that users are primarily academic researchers, state health department staff, and injury prevention practitioners working in non-governmental organizations. WISQARS is also sometimes used by the media, policymakers, and the general public.

TITLE OF INFORMATION COLLECTION: Provide the name of the collection that is the subject of the request. (e.g., Comment card for soliciting feedback on xxxx)

PURPOSE: Provide a brief description of the purpose of this collection and how it will be used. If this is part of a larger study or effort, please include this in your explanation.

DESCRIPTION OF RESPONDENTS: Provide a brief description of the targeted group or groups for this collection of information. These groups must have experience with the program.

TYPE OF COLLECTION: Check one box. If you are requesting approval of other instruments under the generic, you must complete a form for each instrument.
