Dear SOL Participant,
Together we are making history! You are
part of a very important study that
investigates and identifies the diseases
and conditions that affect the Hispanic/
Latino populations that reside in the
United States. With your help we are
learning more about the cardiovascular
disease and other health conditions in
our communities at the Bronx, Chicago,

invitation to come to the Bronx research center at 1 Fordham Plaza for a second visit.
This second visit will be shorter than the first one. You will receive the results of several
of the health exams.
The HCHS/SOL team will be happy to answer any question you might have about the
study. If you have not heard from the HCHS/SOL staff, please contact us at 718.584.1563
to update your information. We do not want you to miss this opportunity to participate in
visit 2. You will be contributing to the understanding of the health of future generations
