Dear DCCPS Fellows:

I would like to gather your feedback about your experiences as a fellow within DCCPS and your input on possible trainings and other resources to support you during your fellowship. Please see below a link to a short on-line survey that requests this information. All the information is anonymous and it should take no more than 5 minutes to complete.

For the cancer prevention fellows, I realize that you may have answered some of these questions before in a previous survey. I ask that you also complete this survey so that I have complete information for all DCCPS fellows.

Please complete the survey no later than November 9th.
