INSTRUCTIONS FOR THE STATE JOHNE’S DISEASE CONTROL PROGRAM ANNUAL REPORT

1. State: Two letter State postal abbreviation.

2. Date range of the report: First and last dates of the calendar year.

DESIGNATED JOHNE’S COORDINATOR

PURPOSE: To provide the current contact for each State. If there is a co-coordinator, list him/her along with his/her information, too.

3. Name: The name of the current designated Johne’s disease coordinator.

4. Address: Mailing and shipping address for the current designated Johne’s disease coordinator.

5. Phone: Phone number for the current designated Johne’s disease coordinator.

6. Email: Email address for the current designated Johne’s disease coordinator.

7. State: Johne’s Disease Website: Web address for the State’s Johne’s disease information, if available.

PROGRAM ACTIVITY

8. Dates of State Johne’s Working Group Meetings: List any dates that the State had a Johne’s working group meeting or cattle health advisory meeting where Johne’s disease program issues were on the agenda.

9. Number of Educational Programs for Producers: Number of educational programs conducted for producers.

10. Number of Educational Programs for Certified Veterinarians: Number of educational programs conducted by the State to certify/recertify veterinarians to work with the VBJDCP.

11. Number of Certified Veterinarians: Number of veterinarians certified/recertified to work with the Johne’s disease control program.

12. Total Number of Education Herds: This is a running total, not specific to the current reporting period. This includes herds that are in a formal education status of the State programs or herds with at least one herd risk assessment on record.

13. Number of Johne’s Management Level Herds: The number of herds enrolled in the management level of the VBJDCP at the end of the current reporting period.

14. Number of Johne’s Program Level 1, 2, 3, or 4 Herds: The number of herds enrolled in the test negative component of the VBJDCP at the end of the current reporting period.

15. Number of Johne’s Program Level A Herds: The number of herds enrolled with a level “A” classification in the VBJDCP at the end of the current reporting period.

16. Number of Johne’s Program Level B, C, or D Herds: The number of herds enrolled with a level “B”, “C”, or “D” classification in the VBJDCP at the end of the current reporting period.

17. Number of Dairy Cows Enrolled in the VBJDCP: The total number of cows enrolled in the VBJDCP summarized from the risk assessment documents.

18. State Dairy Cow Population: Estimate of the cow population in the State. To be counted as a cow, it needs to have calved in the past 12 months or be lactating.

19. Number of Beef Cows Enrolled in the VBJDCP: The total number of cows enrolled in the VBJDCP summarized from the risk assessment documents.

20. State Beef Cow Population: Estimate of the cow population in the State. To be counted as a cow, it needs to have calved in the past 12 months.

NON-FEDERAL SUPPORT TO THE VBJDCP:

21. State Line Item (direct program support): Funding to the Johne’s program as a result of State legislative funds.

22. Personal (in-kind) Support: State support of the program in the way of personnel time committed to the program but not reimbursed using Federal dollars or included in funding support in number 21.

23. Producer Support (testing or vaccine costs): Cost to the producers to participate in the program including cost of testing not covered by State or Federal funding and vaccinations costs.
