Attachment 3: Email to Participate in the HPV Quarterly Reports Satisfaction Survey
Dear immunization program managers,
To date, CDC has sent 3 quarterly HPV vaccination reports to you and health leadership in your state. The quarterly reports include data and key recommendations to continue to facilitate collaboration between awardees and partners to increase HPV vaccination coverage. Previous quarterly reports have focused on reducing missed opportunities, the importance of the provider recommendation, and ACIP’s 9vHPV vaccine recommendations.
CDC would like to better understand how you have used the quarterly reports in your work. Because you work directly in your jurisdiction to implement programming to increase HPV vaccination coverage, your opinions are very important and will help CDC improve the content of future reports. Participation is voluntary and the survey is estimated to take about 8 minutes to complete.
