2015 Immunization Awardee Meeting (IAM) Survey
Dear 2015 Immunization Awardee Meeting participant,
The Immunization Services Division staff at CDC would like to thank you for participating in the Immunization Awardee Meeting held in Atlanta, July 14th-17th, 2015.
This meeting was the first of its kind, and we are very interested in hearing your thoughts! Therefore, we request that you complete this short evaluation of the meeting. The aggregate results will be used by CDC staff to plan future meetings. Your participation is voluntary and the survey is estimated to take about 8 minutes to complete.
