Immunization Services Division All-Awardee Call Evaluation
Dear Immunization Services Division (ISD) All-Awardee Call participant,
The Immunization Services Division staff at CDC would like to thank you for participating in the ISD All-Awardee Calls held February, April, June and August 2015.
These calls were implemented at your request to hear from the entire Division and we are very interested in hearing your thoughts! Therefore, we request that you complete this short evaluation of the calls. The aggregate results will be used by CDC staff to plan for upcoming calls.
