B. Collections of Information Employing Statistical Methods
No statistical methods are used in this data collection.
The information to be collected in the proposed request will be gathered primarily by state and local health department officials and cruise ship physicians/managers. This information will be requested by CDC in the event that persons who have traveled by air or maritime conveyance are exposed to a communicable disease of public health concern. The frequency of these requests will be determined by the number of disease exposure risks that require contact investigation.
Submission is by secure fax or through electronic mail of the appropriate contact investigation outcome form. A copy will be sent to CDC for use in analyzing, coordinating, and maintaining control over the situation that caused imposition of the regulatory requirements.
Response to this data collection is voluntary. The information requested on each of the forms has been streamlined to ensure the ease of response and to minimize the public burden.
CDC currently collects this data under a previously approved data collection. The protocols and electronic systems used for this data collection are continually updated and improved for quality of data collection and ease of use for the public, industry and CDC program administrators.
