Terms of Clearance

This collection is cleared with the following terms of clearance: SSA must report this collection in OMB’s annual Information Collection Budget.

SSA Response

In SSA’s December 20, 2013 letter to the Deputy Administrator, Office of Information and Regulatory Affairs, transmitting SSA’s Information Collection Budget (ICB) Report for 2013 and 2014, we listed this information collection as a Paperwork Reduction Act (PRA) violation (i.e., collection in use without OMB approval).

Revision to the Collection Instrument

Upon OMB approval, we will expand this information collection to include an internet version (fillable, fileable, signable) within the next 3-year renewal cycle.


Creating a Different Modality of Your IC - In this information collection request we are requesting clearance of a newly expanded web-based application version of Form SSA-120. The application will allow first-line supervisors to submit requests through a web-based user interface identical to the paper form layout. Submitted requests are then forwarded to authorized security officials via email for approval. The application will process requests for the 1) release of new user PINs; 2) PIN renewals; and 3) transfers of PINS to new components. The application is planned for agency-wide production rollout summer 2014, subject to OMB approval.


We are revising the PRA statement to reflect our current boilerplate language.  The current language, which dates back to the last reprint of the form, is now outdated.
