Revisions to the Collection Instrument
SSA is making the following revisions:
Change: We are changing the address for the Division of Earnings Records Operations (DERO).
Justification: DERO moved to a new location. Therefore the correct address is shown.
Change: We are removing the information regarding missing persons and letter forwarding from the form.
Justification: SSA no longer offers these services.
Change: We are updating the Paperwork Reduction Act Statement burden information from 3 minutes to 7 minutes
Justification: Based on SSA data, 7 minutes more accurately reflects respondent completion times.
The changes to the online form will go into effect upon OMB approval. At that time, we will place the updated form on SSA’s web page and will remove the existing form.
Other Minor Revisions to the Collection Instrument
SSA’s Office of the General Counsel is conducting a systematic review of SSA’s Privacy Act Statements on agency forms. As a result, SSA is updating the Privacy Act Statement on the the form.
