Dear Renita and Chris:
Subject: MIS - Monitoring and Reporting System for Chronic Disease Programs 0920-0870 (Revision)
I have reviewed this project and determined that it does not involve research with human subjects. IRB
approval is not required.
The OMB package titled “Monitoring and Reporting System for Chronic Disease Prevention and Control
Programs” (Revision-OMB Approval Number 0920-0870) requests approval to continue the collection of
progress and activity information from awardees funded by CDC cooperative agreement DP09-901 during the
last year of the project period. Progress and activity information is collected from awardees through the current
electronic, Web-based Management Information System (MIS) in order to document, monitor, and evaluate
progress toward accomplishing the objectives defined in their work plans. The MIS is used to collect
information for the purposes of performance monitoring and identifying technical assistance needs and does
not constitute human subjects research.
