Upon OMB’s approval, SSA will provide the following Privacy Act Statement to applicants
who have face-to-face or telephone interviews with field office personnel using the SSNAP
Screens:
Privacy Act Statement
Collection and Use of Personal Information
Sections 205(c) (42 U.S.C. 405(c)) and 702 (42 U.S.C. 902) of the Social Security Act, as
amended, authorize us to collect the requested information. We will use the information you
provide to assign you a Social Security number and issue a Social Security card. The Privacy
Act (5 U.S.C. & 552a(b)) permits us to disclose the information you provide in accordance with
approved routine uses. Giving us this information is voluntary; however, failing to provide us
with all or part of the information could prevent us from issuing you a Social Security number
and card. Additional information regarding applying for a Social Security card, our routine uses
of your information, and other Social Security programs, is available on our Internet website,
www.socialsecurity.gov, or at your local Social Security office.
