Summary of Changes
Below is a summary of changes that have occurred since the Supporting Statement and Federal
standard notice templates were posted for the 30-day comment period.

Updated references to regulation and guidance documents in supporting statement
Addressed comments received during 30-day comment period
Updated burden estimates to reflect the simplified discontinuation notices for the
individual market
Updated supporting statement and attachments to reflect that issuers in the small group
market may use the draft Federal standard small group notices released in the June 26,
2014 bulletin, or any forms of the notice otherwise permitted by applicable laws and
regulations
Revised Federal Standard Notices (Attachments 1-6)
o Updated notices to address public comments received during 30-day comment
period, as well as feedback from consumer testing
o Added name, address, and date fields at the beginning of each notice
o Split discontinuation notices to reduce variable language
o Added variable language in the non-QHP renewal notice (Attachment 1) for
grandfathered plans with non-calendar plan years
o Revised QHP notices (Attachments 2 and 4) to emphasize the importance of
updating Marketplace application information by December 15
o Added variable language in QHP discontinuation notices (Attachments 4 and 6) to
account for the situation in which an enrollee’s current Marketplace coverage is
available for renewal only outside the Marketplace
o In the “What if I want to change plans?” section, revised language and headings
to help consumers more clearly identify their particular situation and
corresponding actions
o In the “Questions” section, added a field for issuers to insert their website; listed
agents and brokers and LocalHelp.HealthCare.gov as additional consumer
resources; and included a statement that the notice is available in alternative
formats to persons with disabilities
o Updated and revised the format of the instructions for completing the notices
