Purpose of this Submission: This submission is being made for proposed non-substantive revisions to an existing information collection pursuant to 44 U.S.C. § 3507. This submission seeks to revise FCC Forms 465, 466, and 467, and the accompanying Instructions to reflect a change in filing information for the aforementioned Forms. Please see the attached forms, with the proposed revisions shown in redline.
Summary of Proposed Revisions
Filing Address Changes: Information on filing the forms will be updated to reflect the change that filers must submit Forms 465, 466 and 467, and the accompanying instructions through an online web portal. Current submission methods for Forms 465, 466 and 467 referenced at the end of the forms and for the accompanying instructions referenced under “Where to File” will be replaced with the web address for the Universal Administrative Company’s (USAC) online filing portal.
References to the current submission methods throughout the Forms 465, 466 and 467, and the accompanying instructions, will be replaced with the USAC online filing portal.
Supplement information on For-Profit Hospital Emergency Department and Part-Time Rural Health Clinic will be removed from the instructions for the Forms 465 and 466. This information is no longer accurate and not necessary for filing.
Website Update: On the Form 465 Instructions, the web address for the Rural Areas Search Tool will be replaced with the address to a new website where the tool is now located.
Contact Number: References to the current contact telephone number will be revised to reflect the new contact telephone number.
Date Changes: In Block 3: Funding Year Information, of Forms 465 and 466 allows an applicant to indicate the year for which the applicant seeks funding. The current Forms refer to funding years 2013, 2014 and 2015. Each of the Forms will be revised to refer to funding years 2014, 2015 and 2016.
Stylistic Changes: References to the “Forms Filing Window” will be replaced with “Initial funding request filing period” or “Initial Filing Period;” and “call” will be replaced with “contact;” and “attach” will be replaced with “include;” and “sheets” will be replaced with “information as supporting documentation” for increased clarity. Requests that materials be “mailed” to USAC will be revised to request that materials be “submitted” to reflect the use of online submission. “Enclosed documentation” will be replaced with “Documentation required” to specify that the documents are mandatory.
