such violation if you file a written request for such hearing in the local office of the Natural Resources Conservation Service or
Administering Agency at:

date and place set for the hearing. You may be represented at such a hearing and will be given a full opportunity to present facts and
information relevant to the alleged violation, including oral or documentary evidence. If you fail to request a hearing within the time
specified above, you will have no further right to a hearing.
Request for information concerning this notice or the alleged contract violation should be referred to the above-mentioned NRCS or
Administering Agency office.

PRIVACY ACT AND PUBLIC BURDEN STATEMENT

The following statement is made in accordance with the Privacy Act of 1974 (5 USC 552a) and the Paperwork Reduction Act of 1995, as amended. The
authority for requesting the following information is 7 CFR 631(GPCP); 7 CFR 632 (RAMP); 7 CFR 634 (RWCP); 7 CFR 636 (WHIP); 7 CFR 702
(CRSCP); 7 CFR 752 (WBP); 7 CFR 631 and 702 (IEQIP). The information will be used to notify a program participant of the indication of a contract or
agreement violation. This information may be provided to other agencies, IRS, Department of Justice, or other State and Federal Law enforcement agencies,
and in response to a court magistrate or administrative tribunal. The provisions of criminal and civil fraud statutes, including 18 USC 286, 287, 371, 641, 651,
1001; 15 USC 714m, and 31 USC 372, may be applicable to the information provided.
According to the Paperwork Reduction Act of 1995, an agency may not conduct or sponsor, a person is not required to respond to, a collection of information
unless it displays a currently valid OMB control number. This valid OMB number for this information collection is 0578-0013. The time required to
complete this information collection is estimated to average 49.5 minutes per response, including the time for reviewing instructions, searching existing data
sources, gathering and maintaining the data needed, and completing and reviewing the collection information.
