Dear Resident:


The Food and Drug Administration (FDA) is conducting an important study about food safety in the United States. The study is a telephone survey that asks about people’s opinion on food safety issues and how people handle certain foods. The results will be used to develop educational materials for the American people and will also give us information for our food safety programs.


Your telephone number was randomly generated by a computer to be called for this study. An interviewer will call your house within the next several weeks to ask an adult (aged 18 and over) to participate in this study. Your response is critically important to the accuracy of the study. No other household can substitute for yours because your opinions will represent the views of thousands of families in your area who were not chosen for this study.


Your participation is voluntary. The information you give us will be kept secure to the extent provided by law. FDA will never even know who you are because the interviews do not link your responses with any other identifying information.
