You have been identified as having received training and/or technical assistance (TA) during the past 30 days for your cooperative agreement with the Centers for Disease Control and Prevention (CDC) Division of Community Health (DCH). We are requesting that you follow the link in this email and complete a survey about the training or TA services you received. The purpose of this survey is to assess the accessibility, usefulness, utilization, and quality of training and TA services provided to DCH awardees.
Completion of this survey is voluntary, and strongly encouraged. It will help DCH serve you more effectively by ensuring that the training and TA services you receive meet your needs. Your responses will be maintained in a secure manner by our contractor, RTI International. Individually identifiable information will not be provided to DCH staff, and survey responses will be reported in aggregate.
