You recently received an invitation to complete a survey regarding training or technical assistance you’ve recently received for your cooperative agreement with the Centers for Disease Control and Prevention (CDC) Division of Community Health (DCH).
Completion of this survey is voluntary but strongly encouraged. The results will help DCH serve you more effectively by ensuring that the training and TA services you receive meet your needs and preferences. Your responses will be maintained in a secure manner by our contractor, RTI International. Individually identifiable information will NOT be provided to DCH staff.
