Title: Application for Permanent Employment Certification
Justification: On November 19, 2014, the Office of Management and Budget approved a revision to control number 1205-0451 for form ETA-9089, Application for Permanent Employment Certification, but requested that the Employment and Training Administration (ETA) of the U.S. Department of Labor submit screenshots of the electronic version of the form incorporating the OMB approved changes to the ETA-9089 prior to launching the modified form. Those screenshots are now available and ETA seeks permission to launch both the PDF and electronic versions of the modified form.
