I hereby attest that I have acquired an Import Permit from the
Animal and Plant Health Inspection Service (APHIS), USDA to
import hatching eggs and inedible egg products.

Section D – Agreement for Temporary Transfer

Refused Entry – Cause and Disposition:

IN CONSIDERATION of the U.S. Collector of Customs granting
me (us) permission to transfer temporarily the products described in
Section “C” which are offered for entry into the United States. I
(WE) AGREE under bond filed with said Collector of Customs and
subject to the penalties prescribed in laws enacted by Congress and
regulations issued thereunder by the Secretary of the Treasury, to hold
the said products intact at the location indicated to the right until they
have been inspected and passed by a Livestock, Poultry and Seed
Program Representative or have been otherwise disposed of under the
supervision of a U.S. Customs Officer or a Livestock, Poultry and
Seed Program Representative.

INSTRUCTIONS FOR COMPLETION AND DISTRIBUTION: Importers or their agents may obtain this form by contacting the
Livestock, Poultry and Seed Program offices website at: http://www.ams.usda.gov/LPS

3. If the shipment is refused entry, document the cause and
disposition. Mail the completed form to the LPS office
listed to the left of this form and notify the U.S. Customs
and Border officials.

3. The issued Import Request, Form LPS-222 will be returned
by mail. Alternate arrangements to return the Import
Request can be established. The issued Import Request must
be presented to U.S. Customs and Border Protection
officials for conditional release pending inspection.
4.The importer or agent responsible for the shipment must
provide LPS, USDA, with a contact at the shipment
destination location. An LPS representative will schedule
an inspection of the product and applicable import
documents.

Follow the guidance provided on the Customs and Trade
Automated Interface Requirements (CATAIR) website listed
below for submission of an Import Request through the ACE
system (Automated Commercial Environment):

Livestock, Poultry and Seed Representative
1. When the Import Request submitted is acceptable, enter in
the shell egg affidavit number issued (located at the top
right-hand corner of the form).

* AMS jurisdiction for inspection is limited to eggs
originating from the domesticated chicken, turkey, duck,
goose, or guinea.

2. Upon arrival, complete inspection of the product and import
documents. If acceptable for entry, sign and date Section E.
3. When refused entry, record the appropriate information in
the bottom portion of Section E. Inform the U.S. Customs
and Border Protection official(s) at the point of entry.
According to the Paperwork Reduction Act of 1995, an agency may not conduct or sponsor, and a person is not required to respond to a
collection of information unless it displays a valid OMB control number. The valid OMB control number for this information collection is
0581-0113. The time required to complete this information collection is estimated to average .33 hours per response, including the time for
reviewing instructions, searching existing data sources, gathering and maintaining the data needed, and completing and reviewing the
