I am writing to remind you about an important survey Mathematica Policy Research and SRA International are doing for the Centers for Disease Control and Prevention’s National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP), Division of Heart Disease and Stroke Prevention (DHDSP).
