Subject: Request for Participation – WISEWOMAN Program Survey

Dear [FIRST NAME]:

As a WISEWOMAN grantee, your perspectives regarding the program are very important. In order to help us understand your experiences, as well as those of other staff that work in WISEWOMAN grantee and partner agencies, we are writing to request your participation in a survey about your WISEWOMAN program’s protocols and activities. This survey will provide important information to support the national evaluation for the program. Mathematica Policy Research and SRA International are conducting the study on behalf of CDC.

The survey should take 60 minutes to complete. Participation in the survey is completely voluntary. If you do choose to participate, your responses will be kept private. They will be combined with the responses of other grantees and written up in a report to the Centers for Disease Control and Prevention (CDC). Researchers might also use the information from the study in journals, books, or presentations. However, nothing will be said about you as an individual and no individual names will be reported. There are no consequences to you if decide not to participate.
