Subject: Reminder – WISEWOMAN Program Survey

I am writing to remind you about an important survey Mathematica Policy Research and SRA International are doing for the Centers for Disease Control and Prevention’s National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP), Division of Heart Disease and Stroke Prevention (DHDSP). We are asking all WISEWOMAN grantees to complete the survey in order to help us better understand how your program operates and improves the cardiovascular health of women.

While participation in the survey is voluntary, we really would like to hear from you to ensure that we get a complete picture of how WISEWOMAN is being administered by different grantees and how cardiovascular services to women can be improved.

If you have not yet started the survey, you can respond by filling out the attached PDF. When you are done, you can send the completed survey to [email protected]

The questionnaire takes about 60 minutes to complete, and your answers will be kept completely private.
