County Health Policy Implementation Assessment





INTRODUCTION

Before we get started, please review the following information about this study.

  • The County Health Policy Implementation Assessment (CHPIA) is funded by the National Center for Chronic Disease Prevention and Health Promotion at the Centers for Disease Control and Prevention (CDC). RTI International is the data collection contractor conducting the assessment for CDC. RTI is an independent, nonprofit research institute headquartered in Research Triangle Park, NC.

  • We are working with local county health departments in Washington and North Carolina. You were identified as someone who can answer questions about your health department.

  • Your participation in this assessment is voluntary.

  • Answering questions about a single county health department takes about 20 minutes. If you are reporting on more than one county, we estimate that each additional county will take about 8 minutes to complete.

Please answer each question to the best of your ability. We expect that one local health department employee, such as a department head or chronic disease director, should be able to answer questions on behalf of the department. If you find that you are not the best employee to provide a response, please give this questionnaire to the employee who can best answer these questions.
