We are requesting feedback from you in your role coordinating school-based strategies for the State Public Health Actions to Prevent and Control Diabetes, Heart Disease, Obesity, and Associated Risk Factors and Promote School Health (1305 cooperative agreement) or Promoting Adolescent Health Through School-Based HIV/STD Prevention and School-Based Surveillance (1308 cooperative agreement). As you know, the Centers for Disease Control and Prevention (CDC) develops resources for professionals working with schools and school districts to create healthier school environments. Currently, we are seeking your feedback on the following resources:

  • Comprehensive School Physical Activity Program Guide,

  • Health Education Curriculum Analysis Tool,

  • Parents for Healthy Schools Guide, and

  • School Health Guidelines to Promote Healthy Eating and Physical Activity.


We would greatly appreciate your completion of an online assessment that will take approximately 25 minutes. Your feedback is valuable and will be used to refine our school health resources to better meet your needs. We would appreciate your feedback regardless of whether you have used these tools.
