Who must comply?	Coast Guard Auxiliary members wishing to voluntarily participate in the District Five Auxiliary Safety Survey.
What is this collection about?	This information collection is associated with Safety and Risk Management (SARM) Council initiatives to enhance operational safety throughout D5. The SARM Council plans to utilize an online survey to conduct a safety/hazard assessment for organizational volunteers engaged in Coast Guard operations.

When must information be submitted to the Coast Guard?	The deadline for submission will be 15 days after issuance of the survey to Auxiliary members.

What happens when complete information is received?	The D5 SARM Council will work in concert with Auxiliary leadership throughout the district to respond to safety concerns to enhance operations for each member.
