2014 Report of Organization, NC-99001, and 2014 Annual Survey of Manufactures,
MA-10000(L). To access the electronic questionnaires, please login through our Business Help
Site, econhelp.census.gov/cosasm. Response is required within 30 days of receipt.
We created the electronic reporting system to provide a cost-effective and user-friendly method
to collect the Report of Organization and Annual Survey of Manufactures questionnaires.
From the Business Help Site, you can access online services, view sample questionnaires and
instruction manuals, and get answers to your questions. See the reverse side of this letter for
general information.
The Report of Organization is used to update the Census Bureau's list of businesses and provides
key source data for County Business Patterns and other statistical series. Companies, business
analysts, and trade associations use this information in planning investments, production, and
marketing. The Annual Survey of Manufactures provides data about manufacturing that are
crucial to accurate estimates of domestic production and productivity, and to making sound
decisions on economic trade policies.
Title 13, United States Code, Section 182 authorizes the Census Bureau to collect the information
in these surveys; Section 224, makes your response required or mandatory, and Section
9 guarantees that your response is confidential and will be used for statistical purposes
only. Applicable provisions of the law are provided on the Business Help Site. You may use
reasonable estimates if book figures are not readily available.
