You are being invited to participate in a telephone interview with the ICF International on behalf of the Centers for Disease Control and Prevention (CDC) and the Division of Diabetes Translation. Telephone interviews are part of a comprehensive assessment of the National Program to Eliminate Diabetes Related Health Disparities in Vulnerable Populations, a 5-year cooperative agreement that aims to address complications and health disparities associated with type 2 diabetes in vulnerable populations.


You have been asked to participate in an in-depth telephone interview because of your role (e.g. grantee staff, site coordinator, community partner/coalition member) in the Vulnerable Populations project and the valuable insights and perspectives you can share. We plan to use your input to improve information and programs that address the management and control of type 2 diabetes. The information you provide will be important in determining how CDC can better educate and be a resource for individuals living with type 2 diabetes in vulnerable communities.


The topics covered in the interview will include


  • involvement in planning process

  • training and technical assistance

  • capacity building

  • facilitation of interventions

  • impact of the project

  • future impact

  • lessons learned


The telephone interview will require up to 90 minutes. In the next two weeks an ICF staff member will contact you via email to identify the best date and time to conduct the interview. Should you have any questions please feel free to contact me.
