Last week you received a letter from Dr. John Howard, Director of NIOSH, encouraging your participation in a national survey of current and former members of the National Occupational Research Agenda (NORA) [NAME OF SECTOR] sector council. You have been selected to participate in this brief survey about your experiences as a sector council member during the second decade of NORA (2006 – 2016).Your feedback is very important to the success of the formal evaluation of the second decade of NORA, and your input will help develop the third decade (2016 – 2026).

This information collection is authorized in Section 20(a)(1) of the Occupational Safety and Health Act (29 U.S.C. 669). Your participation is completely voluntary and there will be no adverse effects for you if you choose not to participate. Your responses will be secured and not disclosed unless otherwise compelled by law. All survey results will be reported in aggregate form, except for some narrative information that may be summarized, or quoted in whole or in part. All identifiers will be removed from narrative text. Completing and submitting this survey will imply your consent.
