Introductory Email/Study Information Sheet

for CBO/HWC Survey Respondents

Dear <Potential survey participant’s name>,

<School District Name> is participating in a project funded by the Centers for Disease Control and Prevention’s (CDC), Division of Adolescent and School Health (DASH) related to sexual health and social services, referrals, and school climate and safety. This project focuses on meeting the needs of lesbian, gay, bisexual, transgender, and questioning (LGBTQ) youth and, in particular, males who identify as gay or bisexual, or engage in sexual activities with other males. ICF International, a research and evaluation firm, is helping the district to better understand the impact that the project activities have on meeting the needs of these youth.

As a staff member of one of the community-based organizations (CBO) or health and/or wellness centers (HWC) that the district has identified as a partner, you are being asked to help us learn about your organization and your organization’s relationship with participating schools. Staff from up to 10 CBOs and 10 HWCs in your community are being asked to complete this 40-60 minute survey. The survey contains questions about the services offered by your organization and your organization’s relationships with the school district and participating schools.

We encourage you to consult with other staff, as needed, to complete the survey if there are certain questions you feel would be best answered by another staff member. Attached you will find a copy of the survey which you can print out and complete before entering them in the web survey, if desired.

Your participation in this survey is voluntary, and if you chose to participate, your responses will be kept confidential. The <School District Name> will receive a summary report, but this report will not link your responses to your name or your CBO/HWC’s name without your permission.
