Summary of NSFG Questionnaire Changes for 2015

Key inputs for 2015 changes:

  • Nov 2013 Advisory Workshop focused on revisions related to Affordable Care Act

  • Apr 2014 Advisory Workshop focused on other topic areas and design aspects of the survey

  • Questionnaire revisions deferred in past years for consideration in 2015, largely based on prior input from cosponsoring agencies and from prior NSFG Research Conferences



New or Revised Items for Male Questionnaire (* as asked for females)


  • Extent of pregnancy mistiming for men’s biological children reported as coming “too soon” *

  • Relationship quality with mother of his nonresidential (focal) child

  • General risk of cancer associated with alcohol use*

  • Current cigarette smoking *

  • Correctness of condom use*

  • Global question on any other same-sex experience*


New or Revised Items for Female Questionnaire


  • IUD type for last method (2013 ever, discontinued, and recent)

  • Non-contraceptive reasons for recent hormonal IUD use

  • Timing of birth control method discontinuation (last 12 months)

  • Source, insurance & payment for selected birth control methods currently in use

  • Whether would switch/start using birth control if cost not a factor

  • Whether seeking pregnancy with recent partner(s)

  • Preventive services screening/counseling items added on:

    • LARCs

    • Folic Acid consumption

    • Clinical breast exam

  • Modifications and updates to breast cancer risk series

  • Military service and sheltered homelessness (as asked for males)


New or Revised Items for Both Males and Females


  • Reordering of household roster and marital status

  • Location follow-ups for two sex education items (SEDBC & SEDABST)

  • Health insurance series and follow-up questions modified to match NHIS

  • Whether purchased plan on the exchange

  • Whether pay a premium for plan bought on the exchange

  • Preventive services screening/counseling items added on:

    • Obesity/BMI

    • Blood pressure/hypertension

    • Smoking cessation

  • Relationship status at time of 1st sex with same-sex partners

  • Sexual orientation question “crossover” study to compare NHIS and NSFG questions
