The data tables in this Attachment are designed to assist the EQRO in conducting Protocol 4 for
validation of encounter data submitted by the MCO.

Validation (p. 1) and the Data Field Validation (p. 2) onto the Medical Record Results Summary
Sheet (Attachment II).
