Changes to VA FORM 40-10088 REQUEST FOR REIMBURSEMENT OF CASKET/URN

The National Cemetery Administration (NCA) makes the following changes to VA Form 40-10088, Request for Reimbursement of Casket/Urn. This form has been assigned Office of Management and Budget control number 2900-0799.

In Section II of VA Form 40-10088 (page 2), we are updating the reference to the vendor form that claimants must submit for payment enrollment. This change is necessary because VA recently discontinued use of Standard Form 3881, ACH Vendor/Miscellaneous Payment Enrollment Form and implemented use of VA Form 10091, FSC Vendor File Request Form for payment purposes. We also included minor instructional edits for clarity and to ensure appropriate handling of all requested documentation.

In the section indicated FOR COMPLETION BY VA NATIONAL CEMETERY OFFICIAL (page 2), we are adding different types of cremation urns that cemetery directors must note as acceptable or not acceptable for burial purposes. This change is necessary to address a public comment received during the proposed rule phase to accept urns of various construction. We have added check boxes for VA cemetery officials to indicate whether the cremation urn is made of durable plastic, metal, wood, or other materials and provide space for the official to specify. Cemetery officials will also check whether the urn is acceptable or not acceptable.

The above-referenced changes to VA Form 40-10088 present no additional burden on respondents and are considered non-significant.
