To help APHIS prepare for a potential HPAI outbreak this fall, please complete this survey by
August 14, 2015. Data from the survey will help APHIS assist your industry if an outbreak occurs.
Specifically, the survey will ask you to describe steps taken by you to prepare for an HPAI outbreak
this fall. Please provide as much information as possible.
The questions marked with an * must be completed. If you don’t know an answer select “Don’t know”
on the multiple choice questions; on the write-in questions write “n/a” or “Don’t know” if applicable.
