To help APHIS prepare for a potential HPAI outbreak in your State this fall, please complete this
survey. Data from the survey will help APHIS assist your State if an outbreak occurs. Specifically, the
survey will ask you to describe steps taken by your State to prepare for an HPAI outbreak this fall.
The questions marked with an * must be completed in order to move on. If you don’t know an answer
select “Don’t know” on the multiple choice questions and on the write in questions write in “n/a” or
“don’t know” if applicable.
Even though Survey Monkey will allow you to save and return to the survey to edit or complete later,
