The Community Assets / Community Health (CA/CH) research study, funded by the USDA, is currently being conducted in [TOWN] and 149 other communities in the central United States. The goal is to identify ways to help strengthen small rural communities and their local health care industries.


Input from local primary health care providers is essential to this effort. Recently we sent you a letter, brochure, and survey with questions about your perspective and experiences as a health care provider in your local area. We have not yet received a response, and we would value your input as soon as time allows. Completing the survey requires about 15 minutes. For your convenience, the survey is also available online and your personalized link appears below.

This survey is being administered by Iowa State University with the highest confidentiality and information security standards. Information provided will be used only for statistical research purposes, and all information you provide will be kept completely confidential with results presented in summary form only.
