Pretest Protocol for Outlet Survey

[Pilot Grantee] approached outlets implementing FINI.
On behalf of the Food and Nutrition Service, United States Department of Agriculture, Westat, a research company is conducting a national evaluation of the FINI projects. One component of this evaluation involves a survey of FINI outlets. The questions asked in this survey are about how each outlet is implementing FINI.
Please complete the outlet survey and answer the following questions:
When respondent has finished the survey, ask them:

How can it be improved?
