SUBMIT REPORT: Biannually (twice a year) on or before the 10th day of January and July, the permittee must submit this form containing records of each transaction by either e-mail to: [email protected] OR by mail to: Regional Investigative Analyst, U.S. Fish and Wildlife Service, Office of Law Enforcement, 1011 East Tudor Road, Mail Stop 151, Anchorage, AK 99503-6199. If no transactions have occurred, a negative report is required.

1. The gathering of information on fish and wildlife is authorized by:

3. The relevant burden for the report conditioned by an issued Registration of Agent/Tannery is 1 hour. This burden estimate includes time for reviewing instructions, gathering and maintaining data and completing and reviewing the report. You may direct comments regarding the burden estimate to the Service Information Clearance Officer, U.S. Fish and Wildlife Service, Mail Stop 2042-PDM, U.S. Department of the Interior, 1849 C Street, NW, Washington D.C. 20240.
