Narrative of Revisions

The purpose of the Narrative of Revisions is to clearly indicate revisions to a collection since the previous approval

The following listed below are revisions to the collection:


  • Improved use of information technology. The Coast Guard updated the CG-705A to an electronic fillable version that provides respondents the option to complete a CG-705A electronically. The Coast Guard hopes to create a mechanism for the online electronic submittal of the CG-705A via a web portal in the future. The updated electronic fillable CG-705A is estimated to be burden neutral.

    • Expanded form from three pages (14-1/2” x 28-1/2”) to a four pages (8-1/2” x 11”) format to make the form more user friendly and to accommodate electronic users.

  • Changes to the CG-705A form include:

  • Updated Department of Transportation to Department of Homeland Security

  • Updated C.G. 705 A to CG-705A

  • Updated R. S. 4612, as amended – U. S. C., title 46, sec. 713 to 46 United States Code (U.S.C.) § 10302 & 10502.

  • Removed “Notice is hereby given that section 4519 of the U.S. Revised Statutes (U.S.C., title 46, sec. 577) makes it obligatory on the part of the master of a merchant vessel of the United States, at the commencement of every voyage or engagement, to cause a legible copy of the agreement (forecastle card), omitting signatures, to be placed or posted up in such part of the vessel as to be accessible to the crew, under penalty not exceeding ONE HUNDRED DOLLARS” and replaced it with “Notice is hereby given that 46 U.S.C. 10307 makes it obligatory on the part of the master or individual in charge of a vessel when shipping articles are required, to have a legible copy of the agreement required by 46 U.S.C 10302, unsigned by the mariner, and without the next of kin information, exhibited in a part of the vessel accessible to the crew.”

  • Removed “Required by act of Congress, title LIII, Revised Statutes of the United States (U.S.C., title 46, ch. 18).

  • Removed DATE OF REGISTER

  • Removed VOYAGE NO.

  • Removed NUMBER OF SEAMEN AND APPRENTICES FOR WHICH ACCOMMODATION IS CERTIFIED


Scale of Provisions

  • Removed table and verbiage under ‘SCALE OF PROVISIONS’ section.

  • Updated SCALE OF PROVISIONS with requirements outlined in 46 U.S.C. 10303(b) and 46 USC 10302 to include: “A seaman shall be provided at least 3 meals a day that total at least 3,100 calories, including adequate water and adequate protein, vitamins, and minerals in accordance with the United States Recommended Daily Allowances.”

  • Removed “Going on shore in foreign ports is prohibited except by permission of the master, no dangerous weapons or grog allowed, and none to be brought on board by the crew.”

  • Removed ‘CITIZENSHIP REQUIREMENTS’ table.

  • Removed ‘ATTENTION TO THE MASTERS ESPECIALLY INVITED TO THE FOLLOWING REQUIREMENTS OF LAW’ section.


Form of Agreement

Updated form of agreement section to comply with 46 U.S.C. 10304. Updated to: “IT IS AGREED between the Master and seamen of the ________________ , of which _____________ is at present master, or whoever shall go for Master, now bound from the port of _____________ to_________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________

(description of voyage and names of places at which the vessel is to touch, or the general nature and probable length of the voyage is to be stated)

, for a term not exceeding ________ calendar months.


The seamen agree to conduct themselves in an orderly, faithful, honest, and sober manner, and to be at all times diligent in their respective duties, and to be obedient to the lawful commands of the master, or of an individual who lawfully succeeds the master, and of their superior officers in everything related to the vessel, and the stores and cargo of the vessel, whether on board, in boats, or on shore. In consideration of this service by the seamen to be performed, the master agrees to pay the crew, as wages, the amounts beside their names respectively expressed, and to supply them with the following provisions: A seaman shall be provided at least 3 meals a day that total at least 3,100 calories, including adequate water and adequate protein, vitamins, and minerals in accordance with the United States Recommended Daily Allowances.


It is agreed that any embezzlement, or willful or negligent destruction of any part of the vessel’s cargo or stores, shall be made good to the owner out of the wages of the person guilty of the embezzlement or destruction. If an individual holds himself or herself out as qualified for a duty which the individual proves incompetent to perform, the individual’s wages shall be reduced in proportion to the incompetency.


It also is agreed that if a seaman considers himself or herself to be aggrieved by any breach of this agreement or otherwise, the seaman shall present the complaint to the master or officer in charge of the vessel, in a quiet and orderly manner, who shall take steps that the case requires.

1. AUTHORITY WHICH AUTHORIZED THE SOLICITATION OF INFORMATION:

A. 46 U.S.C. § 10302, 10303, 10304 , 10305, AND 10502.

B. SEE 46 CODE OF FEDERAL REGULATIONS PART 14.

2. PRINCIPLE PURPOSES FOR WHICH INFORMATION IS INTENDED TO BE USED:

A. ESTABLISH A CONTRACT BETWEEN THE SHIPPING COMPANY AND THE CREW MEMBERS PRIOR TO THE BEGINNING OF A VOYAGE AS IS REQUIRED BY LAW.

B. PROVIDES THE U.S. COAST GUARD WITH AN OFFICIAL VESSEL ACTIVITY FILE AS WELL AS A COMPLETE LISTING OF THE SEAMEN EMPLOYED DURING THE VOYAGE, THEIR NAMES, WAGES, ADDRESSES OF NEXT OF KIN, TIME TO BE ONBOARD, BIRTH DATES, CAPACITIES, AND CREDENTIAL NUMBERS.

C. PART OF THE INFORMATION IS TRANSFERRED TO A FILE MANAGEMENT COMPUTER SYSTEM FOR A PERMANENT RECORD.

3. THE ROUTINE USES WHICH MAY BE MADE OF THE INFORMATION:

A. TO MAINTAIN RECORDS REQUIRED BY 46 U.S.C. 7319.

B. TO ENABLE ELIGIBLE PARTIES (i.e. the mariner’s heirs or properly designated representative) TO OBTAIN INFORMATION.

C. TO PROVIDE INFORMATION TO THE U.S. MARITIME ADMINISTRATION FOR USE IN DEVELOPING MANPOWER STUDIES AND TRAINING BUDGET NEEDS.

D. TO DEVELOP INFORMATION AT THE REQUEST OF COMMITTEES OF CONGRESS.

E. TO PROJECT BILLET ASSIGNMENTS AT COAST GUARD MARINE INSPECTION/SAFETY UNITS.

F. TO PROVIDE INFORMATION TO LAW ENFORCEMENT AGENCIES FOR CRIMINAL OR CIVIL LAW ENFORCEMENT PURPOSES.

G. TO ASSIST U.S. COAST GUARD INVESTIGATING OFFICERS AND ADMINISTRATIVE LAW JUDGES IN DETERMINING MISCONDUCT, CAUSES OF CASUALTIES, AND APPROPRIATE SUSPENSION AND REVOCATION ACTIONS.

4. WHETHER OR NOT DISCLOSURE OF SUCH INFORMATION IS MANDATORY OR VOLUNTARY (Required by law or optional) AND THE EFFECTS ON THE INDIVIDUAL, IF ANY, OF NOT PROVIDING ALL OR PART OF THE REQUESTED INFORMATION: DISCLOSURE OF THIS INFORMATION IS VOLUNTARY, BUT SUBJECT SEAMEN MAY NOT BE PERMITTED TO ENGAGE IN EMPLOYMENT ABOARD THE VESSEL FOR WHICH ARTICLES ARE BEING OPENED.
