You recently received an invitation to submit the Year 3 National Diabetes Prevention Program (DPP) Grantee and Intervention Site Excel data collection spreadsheets to the Centers for Disease Control and Prevention (CDC).

Thank you for agreeing to provide responses for this year 3 assessment of National DPP grantee work under funding opportunity announcement DP12-1212PPHF12. Completion of this spreadsheet will inform technical assistance and help to identify best practices for scaling and sustaining the National DPP. If you have not done so yet, we would greatly appreciate you taking the time to complete the grantee spreadsheet, secure a completed spreadsheet from each of your sites, and submit all of these by December 29^th, 2015.
