TITLE OF INFORMATION COLLECTION: NEH Website Evaluation Survey
PURPOSE:
To collect feedback about NEH’s website in order to improve the website’s appearance and
functionality, particularly for potential NEH funding applicants (a core NEH audience). This
effort is part of NEH’s larger website redesign project.
DESCRIPTION OF RESPONDENTS:
Respondents are NEH website users who are also potential NEH funding applicants. Some
respondents may have applied for federal (including NEH), state, local, and foundation grants in
the past.

NEH will survey graduate students at Teachers College, Columbia University. An NEH
employee, who is also a graduate student at Teachers College, will email the survey invitation to
fellow students in her graduate program.

Note: NEH may hold an informal discussion in connection with this survey. After reviewing the
law and OMB guidance, NEH does not consider this discussion to be an “information
collection” subject to the Paperwork Reduction Act. See 5 C.F.R. 1320.3(h)(4); Office of
Management and Budget, Office of Information and Regulatory Affairs, Memorandum re “Social

The purpose of the discussion is to collect free-form feedback about NEH’s website from
members of the public. Although the discussion may address some of the questions the survey
poses, it will be largely unstructured; NEH will not adhere to a script or a pre-established list of

questions, and participants will be able to provide general comments and suggestions in the
manner and to the extent they choose.
Please make sure that all instruments, instructions, and scripts are submitted with the
request.

Instructions for completing Request for Approval under the “Generic
Clearance for the Collection of Routine Customer Feedback”
TITLE OF INFORMATION COLLECTION: Provide the name of the collection that is the
subject of the request. (e.g. Comment card for soliciting feedback on xxxx)
PURPOSE: Provide a brief description of the purpose of this collection and how it will be used.
If this is part of a larger study or effort, please include this in your explanation.
DESCRIPTION OF RESPONDENTS: Provide a brief description of the targeted group or
groups for this collection of information. These groups must have experience with the program.
TYPE OF COLLECTION: Check one box. If you are requesting approval of other
instruments under the generic, you must complete a form for each instrument.
CERTIFICATION: Please read the certification carefully. If you incorrectly certify, the
collection will be returned as improperly submitted or it will be disapproved.
Personally Identifiable Information: Provide answers to the questions. Note: Agencies
should only collect PII to the extent necessary, and they should only retain PII for the period of
time that is necessary to achieve a specific objective.
Gifts or Payments: If you answer yes to the question, please describe the incentive and provide
a justification for the amount.
