Parent-Legal Guardian Questionnaire Distributor Script

Hello (insert name of parent-legal guardian here),

(Insert name of Child Welfare Services agency here) would like you to take part in a study to help them learn if the services you get help children safely stay out of foster care or safely leave foster care sooner. The survey is being conducted by Westat, a research organization in Maryland.

You can participate in the study by completing the survey in this packet. The survey asks questions about the services and support you receive from me, your social worker. It also asks about other family members and friends that may be involved with your case.

You do not have to be in the study and you can stop being in the study at any time. We will keep your information private to the extent permitted by law. For example, even though the survey asks questions about me, I will never see your responses because you can seal the survey in the pre-addressed stamped envelope and put it in the mail. If you can’t get to a mailbox, you can leave it with me, or take it to (insert name of Child Welfare Services agency here) the next time you are there. (Insert name of Child Welfare Services agency here) or I will put it in the mail for you. Your responses cannot be used in court.

You will receive a $50.00 Visa gift card for completing the survey.

Instructions on what to do are contained in the packet. If you have any questions about the survey or would like to take the survey over the telephone, there is an 800 number to call – the staff members at Westat will be happy to help you.

Potential Questions from Parents and Legal Guardians

Will you see my responses?	No. After you complete the survey, you will put it in the pre-addressed, stamped envelope and drop it in the mail.

Can my survey responses be used in court?	No.
I don’t know if I can get to a mailbox. How can I send the survey in?	After putting your completed survey in the pre-addressed, stamped envelope and sealing it, you can give it to me or take it to (insert name of Child Welfare Services agency here). We will put it in the mail for you.
Can you read the survey to me?	No, I can’t be around you when you take the survey. There is an 800 number in the packet, and Westat will be happy to help you complete the survey.
How long does the survey take?	About 36 minutes.
When do I need to do this?	As soon as possible.
Can I get cash or a gift card to another store?	No. The $50.00 Visa gift card is the only option.
When do I get the gift card?	You should get it a few weeks after Westat receives your survey. You may call 1-855-538-6735 if you do not receive the incentive.
How will Westat know where to send the gift card?	There is a space in the survey to write down the address of where to send the gift card. You can include your own name as part of the address or someone else’s name.
Can I take the survey more than once?	No. You can only take the survey one time.
