Change Request Justification, 1219-0007 FORMS: 7000-1 and 7000-2



The following changes were made since the last approval to the below MSHA forms. No changes to the burden or supporting statement result.



7000-1

-The title for Section A is changed now to “Identification Data” instead of “Identification.”

The previous posted form had “Identification D.” with the word Data missing. This change will not change the minutes of the response for completing the form.



-The word company is removed from question 2.

In section B- Question 2, Name of Investigator was on the original form, correcting it back by removing the word Company. This change will not change the minutes of the response for completing the form.



-The selection choice "unknown" was removed under question 5

Selection-Unknown 5 (b) 07 removed to align the form with Part 50 CFR, 50.20-6 (i). Unknown is not an option pursuit to the regulation. This change will not change the minutes of the response for completing the form.



-The word “Quit” and the comma before it is removed from question 28.

The word “Quit” was removed to align the form with Part 50 CFR, 50.20-7 Section D (a) Item 28, Quit is not an option within the regulation. This change will not impact the minutes of the response for completing the form.



-The selection to enter the respondent's email at the bottom of the form was removed.

The email address and line was removed to align the form with Part 50 CFR 50.20-7 Section D where the requirements for the form content comes from. The email address request information was an optional field and was not within the Part 50 regulations at this time. The removal of it has a de minimus impact on the response time in completing the form.

General style non-content changes:

-Capitalization that was more appropriate was added throughout.

-Selection button shapes were changed.

These changes will not impact the minutes of the response for completing the form.



7000-2

-The word “or” was removed between MSHA ID Number and Contractor ID on the bottom right.

The word “or” was removed on the bottom of the 7000-2 form due to the word “or” not being on the original form version and not in the 30 CFR Part 50 regulations from which this requirement comes from. The previous management had added it thinking it would clarify to the public that they would not have both types of ID numbers. However, having both ID numbers listed is rare. If you were a Mine Operator it would be a mine MSHA ID number required. If you were reporting for a contractor they would use the Contractor ID number only, not both. This change is de minimus and will not impact the minutes of the response for completing the form.
