I search and found a Sep 2013 email indicating Cindy Allard, OSD/JS Privacy,
coordinated on the DD Form 67, no PAS required, but I cannot find her email
or the DD Form 67.  I contacted the OSD/JS Privacy Office and asked them if
the form required a PAS.  Ms. Vicki Short, Privacy analyst, said the form
did not require a PAS and coordinated on the attached DD Form 67.  So, as
determined back in Sep 2013 and updated Sep 2015, no PAS is required.
If you need anything else please don't hesitate to get in touch.
