Attachment 12: Email invitation to T2 web-survey participants

Dear Participant,

Last year, you participated in a web-based survey for the National Institute for Allergy and Infectious Disease’s Critical Events Policy Implementation (CEPI) program. Thank you for your participation. The goal of this survey was to establish baseline knowledge of professionals’ awareness, accessibility, understanding, and experience with the DAIDS Critical Events Policy and Manual, determine applicability of the Critical Events Policy and Manual to clinical research, and acquire recommendations to improve critical event reporting. You are receiving this email because we are requesting your participation in an additional survey that will assess changes in these elements now that a year has passed since your last assessment. Your survey responses will be kept secure.

The entire survey is voluntary and will take no more than 30 minutes to complete. I encourage you to participate. Please click here (insert link, see Attachments 18 or 19) to take the short, web-based survey by [date]:

Once you have completed the survey, Social Solutions will send you your stipend [omitted for DAIDS staff, including contractor-DAIDS staff, and Site monitors].
