The completed Notice of Loss (form CCC-576 Parts A and B) must be filed with FSA by the following:
for prevented planting claims, within 15 calendar days after the final planting date
for low yield and value loss claims, the earlier of the following:
15 calendar days after the damaging weather or disaster occurrence
15 calendar days after date damage to the specific crop acreage or commodity is apparent to the producer
15 calendar days after the normal harvest date.
The completed Application for Payment (form CCC-576 Parts D and H) must be filed with FSA no later than 60 calendar days after the coverage period ends for the crop.
Exception: An application for NAP payment for perennial forage intended for grazing will not be required on loss filed based upon independent assessments.
