Subject: Request for Epi-Aid

Hi Krista,
We are requesting technical assistance from CDC in the form of a CDC Epi-Aid investigation to address a recently
reported multi-drug resistant tuberculosis case. The investigation will remain under the direction and supervision of the
Arizona Department of Health Services (ADHS) in conjunction with the local health agency, the Tribe and other
jurisdictions that may be impacted. Further, ADHS will retain ownership of all data collected.
I understand the proposed objectives include:
1) Determine the chain or chains of transmission
2) Identify and prioritize contacts
3) Estimate the scope of transmission
4) Develop a plan for contacts with presumed multidrug-resistant TB infection and ensure that treatment and
evaluation recommendations are made in close consultation with clinical TB experts in the management of drugresistant TB
5) Facilitate communications among involved agencies to assist with the coordination of contact investigations

Eric Hawkins will be the point of contact for ADHS and Dr. Jefferson Jones, EISO assigned to Arziona, will assist
with the investigation. We look forward to working with your team.
