Subject: Reminder: The IPV Provider Network Project Brief Survey is Now Open!

Dear [Service Provider]:

The survey link for the U.S. Department of Health and Human Services, Office on Women’s Health (OWH) Project on Intimate Partner Violence (IPV) Provider Network is now open!  While we anticipate the brief survey will take 30 minutes at most to complete, we encourage you to begin entering data as soon as possible to give yourself sufficient time to do so before the deadline ([Weekday, Month Day]), after which you will be locked out of the system until written authorization for an extension is provided to us by OWH.

As a reminder, you can access the survey to enter your data at the following web address: https://x.norc.org. Per previous notice, please be advised that the open reporting period is limited to a 2-week period.
