Several weeks ago we sent you an e-mail inviting you to complete a voluntary short survey about your participation in the 2016 Science Ambassador Workshop at the Centers for Disease Control and Prevention in Atlanta. We strive to improve the workshop, and are interested in your feedback and suggestions to guide the direction of future workshops.

Please complete the survey by accessing the link below if you have not done so already. The survey should take approximately 20 minutes to complete. Be sure to complete this survey in one sitting, as you will not be able to return to edit your responses or complete the survey once you exit it. The survey closes on Monday, August 8th at 11:59 pm EST.
