The IRB has approved a waiver of parental permission for this study for participation of
minors ages 18 years old in states where the age of majority is higher than 18 years old.
Any amendments or other changes to this protocol must be submitted for review by the
NORC IRB, and all adverse events must be reported to the NORC IRB.
The IRB has approved your research for a period of 12 months. To avoid a lapse in IRB
approval, please submit your continuing review materials to the IRB at least 2 weeks prior
